How do I prove my residence in Brooklyn to obtain a legal name change?

When you file your name change petition in Brooklyn Civil Court, you must prove your residence in Brooklyn so that the court can recognize its jurisdiction over you and grant the legal name change.

The Brooklyn Civil Court will accept the following as proof a resident address before granting the legal name change:

A driver license with a Brooklyn address. 

A New York State non-driver identification issued by the department of motor vehicles with a Brooklyn address.  

A utility bill from gas, electric, cable, and/or telephone company with a Brooklyn address.   A cell phone bill can be used. Although a judge or a clerk in the Brooklyn Court may in their discretion not accept it.  So it’s a good idea to have more than item of proof listed in this blog to prove your Brooklyn residence.  

A bank statement with a Brooklyn address. 

A credit card statement with a Brooklyn address. 

A paystub with your Brooklyn address. 

A letter from a government agency addressed to you at a Brooklyn address .  Examples of  letters from a government agency addressed you would be:   a letter from the Social Security Administration, a letter from the Internal Revenue Service, a letter from NY State Department of Taxation and Finance, a letter from the Board of Elections,   a letter from the Courts requesting you appear for Jury duty, and a letter from the Passport agency.
