IF YOU LIVE IN BROOKLYN, WHO HAS TO BE NOTIFIED OF YOUR NAME CHANGE?

If you live in Brooklyn, who has to be notified of your name change?

In addition to publishing a notice of your name change in a local Brooklyn newspaper, Judges of the Civil Court in Brooklyn may require that you send a copy of the Brooklyn Civil Court name change order and the name change petition to some or all of the following people or entities as applicable:  

a former spouse

a current spouse

the other parent of any child you may have

United States Citizenship and Immigration Services

Department of Homeland Security

United States Passport Agency

Social Security Administration

an employer

in cases involving a child, the child’s school 

the selective service

U.S. Bankruptcy Court

New York State Division of Criminal Justice Services 

Department of Motor Vehicles

any financial institution servicing a student loan
